Telix Pharmaceuticals (US), Inc. (Telix US) is committed to conducting its business in compliance with all state laws that may affect the marketing of our products and our relationship with healthcare professionals, and to this end has established and maintained a Comprehensive Compliance Program (CCP).

Below you will find a description of Telix US’  State-specific program elements related to California’s Health and Safety Code §§ 119400-119402.

Telix US’ Annual Declaration of Compliance for Purposes of California Health & Safety Code §§ 119400-119402

Telix US has developed a Comprehensive Compliance Program (“CCP”) that to the best of our knowledge is in material compliance with the California Health & Safety Code §§ 119400-119402, as well as Telix US’ annual dollar limit for in-scope expenditures with relevant California Health Care Professionals. It is Telix US’  expectation that all employees comply with the Telix Code of Conduct and other applicable policies and procedures that can be found on the Telix Corporate Governance site. Telix US is continuously assessing and enhancing its CCP. Telix US asserts that its CCP, reviewed on June 22, 2026, is reasonably designed to identify or prevent and detect violations of applicable law, regulations and company policies or procedures. A copy of this Declaration may be obtained by telephoning the Telix Hotline tollfree at 1.833.214.1164.

Telix US is committed to conducting business with honesty and integrity. Telix US takes all concerns related to the way it does business seriously, and rigorously investigates all credible issues brought to its attention. As part of this commitment, Telix US protects from retaliation any person who raises a concern about the way that Telix does business.

Telix has a dedicated hotline portal – the Telix Hotline – that includes the option for anonymous reporting. The Telix Hotline is an externally hosted confidential online and telephone helpline available 24 hours a day, 7 days a week. CALIFORNIA PRIVACY RIGHTS NOTICE

This California Privacy Rights Notice (“Notice”) applies to all visitors, users, customers, and suppliers who reside in the State of California (“consumers” or “you”). In the event of a conflict between this Section and the remainder of this Policy, this Section shall take precedence for California residents. Any terms used below and not otherwise defined have the meaning given to them under the California Consumer Privacy Act as Amended by the California Privacy Rights Act and its implementing regulations (the “CCPA”).

The Collection, Source, Purpose and Sharing of Your Personal Information

Telix collects several categories of Personal Information as described above. Within the last twelve (12) months, the Personal Information we have collected consists of the following categories under California law:

  • Identifiers: consisting of name, address, telephone number, email address, social media handles, and usernames;
  • Consumer Information under Cal. Civ. Code 1798.80(e): consisting of information that identifies, relates to, describes, or is capable of being associated with, a particular individual, including, but not limited to, name, social security number, physical characteristics or description, address, telephone number, education, employment, employment history, medical information, and health insurance information;
  • Characteristics of Protected Classifications under California or federal law: consisting of age, race, ethnicity, citizenship, medical condition, physical or mental disability, sex, gender, veteran or military status;
  • Commercial Information: consisting of records of products or services purchased, obtained, or considered, other purchasing or consuming histories or tendencies, payment information, transaction history;
  • Internet or other Electronic Information: such as computer Internet Protocol (IP) address; unique device identifier (UDID), e.g., MAC address; Cookies and other data linked to a device; and data about usage of the website and communications;
  • Geolocation data: consisting of approximate location based on your IP address;
  • Audio, Electronic, and Visual Data: consisting of customer support calls, video recordings, and electronic device information;
  • Professional or employment-related information: consisting of employment history, references, and information you include in your job application or CV;
  • Inferences: inferences drawn to create a profile; and
  • Sensitive Personal Information: consumer’s account log-in, in combination with any required security or access code, password, or credentials allowing access to the account; racial or ethnic origin, personal information about health.

Telix does not knowingly collect or store information about minors under the age of 16, except as legally required or professionally required in our capacity as required by our Services. Telix does not collect, use, disclose, or share Sensitive Personal Information for any purpose other than those purposes for which it was specifically collected or as otherwise permitted by applicable Privacy Laws.

Disclosures

For a Business Purpose

In the last 12 months we disclosed each of the above categories of Personal Information for our business purposes to the following  categories of third parties and / or service providers with whom we disclosed Personal Information in the last 12 months are described above  (Disclosure of Personal Information).

For Selling or Sharing

In the past 12 months, the following categories of Personal Information may have been sold or shared for compensation or cross-contextual behavioural advertising to our vendors that provide cross-contextual behavioural advertising services: (a) identifiers, (b) consumer information under Cal. Civ. Code 1798.80(e), (c) commercial information, (d) internet or other electronic information, (e) geolocation data, (f) audio, electronic and visual data, and (g) inferences. 

We do not have any actual knowledge that we have sold or shared information of any individual under the age of 16 for monetary compensation or for cross-contextual behavioural advertising.

California Consumer Rights

Subject to certain exceptions, residents of California have the right to:

  • request access to specific pieces of your Personal Information;
  • request correction of your Personal Information;
  • request to know what  Personal Information has been collected, including the categories of Personal Information collected, the categories of sources from which such Personal Information is collected, the purpose for collecting, selling, or sharing Personal Information, and the categories of third parties to whom we disclose, share or sell Personal Information;
  • request deletion of your Personal Information; and
  • opt out of the sale or sharing of your Personal Information.

You also have the right to be free of discrimination and retaliation if you elect to exercise any of the above rights. 

You, or your authorized agent, may exercise these rights by (1) calling us via our US Syntrio Ethics English-speaking hotline toll free at +1-833-214-1164 or our US Syntrio Ethics Spanish speaking hotline toll free at +1-800-216-1288, or (2) by emailing us at privacy@telixpharma.com and providing us with your name, email address, relationship to Telix, nature of your request, and state of residence.

You must provide sufficient information to allow us to reasonably authenticate your request and verify you are the person about whom we collected Personal Information or that you are the authorized agent of such person.  We may require additional information to verify and authenticate the request, which may vary depending on the Personal Information we have on file for you and the nature of your request. We will be unable to respond if you do not provide  sufficient detail to allow us to properly understand, evaluate, and respond to your request.

We will strive to respond to your request in a timely manner as required by law.  We may charge a fee to process or respond to your request if it is excessive, repetitive, or manifestly unfounded.

To opt-out online, please contact us as set out in Section 3.11 or opt-out in accordance with the unsubscribe communications on relevant electronic communication.

CONSUMER HEALTH PRIVACY NOTICE

This Consumer Health Privacy Notice (“Notice”) applies to all operations for Telix Pharmaceuticals (US) Inc (“Telix US” or “we”) and supplements the Global Privacy Policy for Telix Pharmaceuticals Limited. It applies solely to health information or health data as defined by Washington and Nevada law (“Consumer Health Information”) to provide additional information about how Telix may process Consumer Health Information, how we protect it, as well as your rights and choices with respect to that information. It applies to all operations for Telix US, as well as its websites, mobile applications, and digital services (“Services”) that link to or post this Notice.
This Notice applies only to residents of Washington and Nevada and to individuals whose information is processed in Washington or Nevada. It does not apply to the personal information we process about Telix workforce members, third party partners, or personal information that is governed by other applicable laws, including protected health information we may process as a business associate under the Health Insurance Portability and Accountability Act (“HIPAA”) of 1996.
Consumer Health Information for purposes of this Notice is defined by Washington or Nevada law and includes information that identifies or is reasonably capable of being associated or linked, directly or indirectly, with a particular consumer and that identifies the consumer’s past, present, or future physical or mental health status.

1. Categories of Consumer Health Information Collected

Telix US may collect and process the following categories of Consumer Health Information:

  • Individual health conditions, treatment, diseases, or diagnoses;
  • Social, psychological, behavioral, and medical interventions;
  • Health-related surgeries or procedures;
  • Use or purchase of prescribed medication or ordered product;
  • Bodily functions, vital signs, symptoms, or measurements of health information;
  • Diagnoses or diagnostic testing, treatment, or medication;
  • Genetic data;
  • Precise location information that could reasonably indicate a resident’s attempt to acquire or receive health services or supplies;
  • Data that identifies a consumer seeking health care services; and
  • Consumer health information inferred or derived from non-health information.

2. Sources From Which Consumer Health Information May Be Collected:

Telix US may collect Consumer Health Information directly from:

  • You, your mobile devices and your computer and other internet-connected devices upon downloading and when you visit or interact with our physical sites, websites, applications, and online platforms;
  • Third parties including joint marketing and advertising partners who provide digital marketing services, website and other digital, marketing or online security services;
  • Publicly available sources including public databases, government sources and other third parties;
  • Other Telix entities;
  • Third parties and processors who provide benefit verification, program, enrollment, and product fulfillment services in connection with our products and services;
  • Third parties and processors who help us maintain the accuracy of our data and data aggregators that help us complete and enhance our records;
  • Third parties and processors that provide access to information you make publicly available, such as social media platforms; and
  • Third parties and processors who assist with fraud prevention, detection, and mitigation.

3. Purposes for Processing Consumer Health Information

Telix US may process the above-mentioned categories of Consumer Health Information for the following purposes:

  • To provide, maintain, and improve our products and services;
  • To process transactions and fulfill requests initiated by you;
  • To communicate with you and provide you support, such as to address your inquiries and other communications you send to us;
  • To operate, evaluate, and improve our business, such as sending you information about our services, performing accounting, auditing, record keeping, and other internal functions, conducting research, developing new products and services, analyzing and improving our sites and services, personalizing your experience, and implementing various business development matters (e.g., mergers, acquisitions, and other business transactions);
  • To comply with applicable legal, regulatory and contractual obligations;
  • To detect, prevent, and respond to security incidents or fraudulent activity; and
  • To verify or maintain the quality or safety of a product, service, or device that is owned, manufactured by, manufactured for, or controlled by us, and to improve, upgrade, or enhance the service or device that is owned, manufactured, manufactured for, or controlled by us.

4. Sharing of Consumer Health Information

We may share each of the above categories of Consumer Health Information with third parties and affiliates such as:

  • Other Telix Pharmaceutical Limited entities;
  • Third-party services providers who perform services on our behalf including third-parties:
    • that help administer, manage, and analyze our programs and services;
    • with whom we have joint marketing and similar arrangements or who assist with our information technology and security programs;
    • who provide marketing and data analytics services, such as social media platforms used to deliver our ads, website/email optimization providers, email marketing vendors, and data analytics vendors;
    • who are network advertising partners;
    • who assist with fraud prevention, detection, and mitigation;
    • who are reasonably necessary to facilitate a merger, sale, joint venture or collaboration, assignment, transfer, or other disposition of all or any portion of our business, assets, or stock (including in connection with any bankruptcy or similar proceedings);
    • who are our lawyers, auditors, and consultants;
    • who are legal and regulatory bodies, as well as other third parties as required by law.

Telix US does not sell your Consumer Health Information or offer it in exchange for consideration without first obtaining your valid authorization to do so.

5. Exercising Your Rights

Subject to applicable exceptions, you may have certain rights regarding our processing of your Consumer Health Information under applicable law. These include the:

  • Right to know and confirm whether we are collecting, sharing or selling your Consumer Health Information
  • Right to withdraw consent for us to collect or share your Consumer Health Information
  • Right to request deletion of your Consumer Health Information
  • Right to correct your Consumer Health Information

You, or your authorized agent, may exercise these rights by (1) calling us via our US Telix Hotline toll free at +1-833-214-1164 or (2) by emailing us at privacy@telixpharma.com and providing us with your name, email address, relationship to Telix, nature of your request, and state of residence.

You must provide sufficient information to allow us to reasonably authenticate your request and verify you are the person about whom we collected the Consumer Health Information or that you are the authorized agent of such person. We may require additional information to verify and authenticate the request, which may vary depending on the Consumer Health Information we have on file for you and the nature of your request. We will be unable to respond if you do not provide sufficient detail to allow us to properly understand, evaluate, and respond to your request.
